Chowchilla Skilled Nursing and Rehabilitation Facility’s rehabilitation program is specially designed to provide the utmost quality and comfort during your short-term stay.  Our physical, occupational and speech therapists provide a motivating setting to encourage the patient to achieve optimal independence. 

Their focus is on preparing our short-term stay residents to go back to independent living in their community.

A three-day stay in the hospital and a diagnosis of any of the following will qualify you for rehabilitation in this skilled care facility:

  • Hip or Knee Replacement
  • Stroke
  • Amputation
  • Frequent Falls
  • Peripheral Vascular Disease
  • Congestive Heart Failure
  • Pneumonia
  • Osteoarthritis
  • Spinal Cord Injury
  • Dehydration
  • Wound Care
  • IV Antibiotics
  • Upper Respiratory Infection
  • Cellulitis
  • Any Fracture
  • Upon discharge from the hospital with orders for skilled care, the patient will be admitted to our skilled nursing facility. Rehabilitation stays usually range from 14 to 100 days.

    An average length of stay is 47 days. During this time the patient’s room and board, medications, meals, x-rays, laboratory tests and physician visits are covered by Medicare and/or supplemental insurance.

    Your loved one will receive up to three hours of therapy per day and 24-hour nursing supervision. The patient can choose one of our skilled nursing physicians who will follow his/her care while here, with visits from the doctor at least once every 30 days. Upon discharge back into the community, our therapists will do a home evaluation to ensure the transition is as smooth as possible.
